Job summary

PACE Southeast Michigan is seeking a Nurse Practitioner to provide primary care to an assigned participant panel, delivering acute and chronic care management under the supervision of the center Physician-In-Charge. The role includes annual/semi-annual assessments, quarterly chronic care visits, and collaboration with the care team.

Qualifications

• Three to five years of progressively more responsible experience as an RN in a clinical setting with geriatrics experience preferred.
• One year of experience with a frail or elderly population.
• Maintain current Michigan NP License in good standing and fulfill CME requirements.
• The NP must comply with TB screenings, infection control, BLS and in-service requirements.
• The NP supports and contributes to nursing education and interdepartmental collaboration.

Responsibilities

• Provide primary care for an assigned panel of participants and manage acute and chronic conditions.
• Perform monthly quota of comprehensive annual and semi-annual assessments.
• Conduct chronic care management visits every 3 months and update care plans as needed.
• Prescribe medications including controlled substances under physician delegation.
• Coordinate specialty and diagnostic referrals with the supervising physician and interdisciplinary team.
• Initiate diagnostic studies with physician approval and oversight.
• Participate in after-hours on-call coverage as needed and attend the monthly education conference.
