Job summary

The Senior Community Care of Michigan PACE in Lansing, MI is seeking a Full-Time Nurse Practitioner to provide primary care to enrollees under the Medical Director's supervision. This onsite role runs Monday–Friday, 8:00a–4:30p, with rotating after-hours on-call and occasional weekends, including local travel to participant homes.

The NP will perform assessments, develop care plans, educate participants and caregivers, and work within the Interdisciplinary Team to meet the needs of frail elders

Qualifications

• RN license in the state of employment.
• Graduate of a school of professional nursing.
• Nurse Practitioner certification by a national certifying organization recognized by the State Board of Nursing.
• Minimum of 1 year experience with frail and elderly population.
• Training or related experience in the assigned job.

Responsibilities

• Oversee residents of VOANS Skilled Nursing Facilities.
• Evaluate residents and provide medical care and treatment.
• Coordinate care with Primary Care Physicians and Medical Directors.
• Perform in-person history and physical on new enrollees.
• Develop a comprehensive plan of care with the IDT and reassess semiannually.
