Job summary

PACE Southeast Michigan seeks a Nurse Practitioner to deliver primary care for an assigned participant panel in Westland, MI, under the supervision of the center PIC. The NP will manage acute and chronic conditions, perform quarterly assessments, and coordinate care with the interdisciplinary team.

Responsibilities include chronic disease management, annual/semi-annual exams, after-hours on-call support, and comprehensive documentation.

Qualifications

• Maintain current Michigan NP License in good standing and CME requirements.
• Experience in geriatrics and primary care preferred.
• Ability to collaborate with interdisciplinary team and supervising physician.

Responsibilities

• Chronic disease management and annual/semi-annual assessments.
• Chronic care visits every 3 months and updates to management plans.
• Coordinate referrals and diagnostic studies with physician oversight.
• Provide patient education and documentation in SOAP format.
