Job summary

TruHealth is seeking an advanced practice provider (APRN or PA) with primary care or internal medicine experience, preferably geriatrics-focused. You will join a team delivering care within nursing homes or assisted living facilities enrolled in Medicare Advantage plans, focusing on improving residents' outcomes and quality of life.

Qualifications

• Master's degree or higher in nursing or related field.
• APRN license or PA certification required.
• 3 years of clinical nursing or rehab experience in geriatrics.
• Experience with Electronic Health Records (EHR).
• Proficiency with Microsoft Office applications.

Responsibilities

• Assess patients' medical and mental health needs and create a proactive care plan.
• Ensure compliance with local, state, and federal clinical guidelines.
• Prescribe medications and order labs and diagnostics.
• Monitor treatment progress and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
