Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatrics focus, to improve outcomes for long-term care residents by collaborating with a team of providers and case managers. You will work primarily in nursing homes or assisted living communities enrolled in Medicare Advantage plans, delivering personalized, coordinated care.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assessing and planning care for residents in long-term care and assisted living communities.
• Ensuring compliance with local, state, and federal regulations related to clinical services.
• Prescribing medications and ordering labs and consultations as needed.
• Monitoring patient response and modifying treatment plans.
• Collaborating with RN case managers and other members of the care team.
