TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to enhance care for long-term residents. You will work within nursing homes or assisted living facilities, delivering coordinated, person-centered health care under a national model.

Qualifications

• APRN or PA licensure required; geriatrics experience preferred.
• Master's degree or higher in a related field.
• At least 3 years of clinical nursing or rehab experience in geriatric populations.
• Experience with Electronic Health Records and basic Microsoft Office.
• Ability to coordinate care with a multidisciplinary team.

Responsabilités

• Assess patients and develop proactive care plans for medical and mental health needs.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, procedures and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of the care team.
