Job summary

TruHealth is seeking an advanced practice provider (APRN/NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to join our team dedicated to improving life for long-term care residents. You will work with providers and case managers to deliver coordinated, high-quality care within nursing facilities and assisted living communities.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in geriatric clinical nursing or rehab.
• Electronic Health Records experience.
• Microsoft Word/Excel/Outlook proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and create a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patients’ response to treatment and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
