Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric focus to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver proactive, coordinated healthcare in nursing homes or assisted living settings.

You will exercise independent judgement to treat acute and chronic conditions while consulting with a supervising physician as appropriate.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications (Word, Outlook, Excel).

Responsibilities

• Assess patients’ medical and mental health needs and create care plans.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, procedures, and consultations.
• Monitor treatment response and modify plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
