Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with a geriatrics focus for primary care in long-term care settings. You will work with a team to provide coordinated health care that improves residents’ quality of life.

You’ll exercise independent judgment, treat acute and chronic conditions, and collaborate with facility partners to implement our care model in compliance with regulations.

Qualifications

• Requires APRN/NP or PA license and a degree from an accredited program.
• Experience in geriatric populations within clinical nursing or rehab.
• Electronic Health Records experience and MS Office proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and create proactive care plans.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and adjust plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
