Job summary

TruHealth seeks an advanced practice provider (NP/PA) with geriatrics focus to improve outcomes for long-term care residents. The role involves collaboration with a team of providers and case managers within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

You will exercise independent judgment to treat acute and chronic conditions, consulting with a supervising physician as needed, while aligning with national care models and regulatory requirements.

Qualifications

• APRN/NP or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ clinical nursing or rehab experience in geriatric populations
• Electronic Health Records experience
• Working knowledge of Microsoft Word, Outlook and Excel

Responsibilities

• Assess patients' medical needs and develop proactive care plans
• Ensure compliance with local, state, and federal regulations for clinical services
• Prescribe medications and order labs, diagnostics and consults
• Monitor patient progress and adjust treatment plans accordingly
• Collaborate with RN case managers as part of an integrated care team

Education

Master's degree or higher in Nursing or Physician Assistant
