Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to join our team delivering care in nursing homes and assisted living facilities under Medicare Advantage plans.

You will assess residents, coordinate care with a multidisciplinary team, prescribe meds, order labs, ensure regulatory compliance, and participate in career growth within a supportive, mission-driven organization that values work-life balance.

Qualifications

• License as FNP/AGNP/AHACNP or PA required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in geriatric care or rehab.
• Electronic Health Records experience; MS Office proficiency

Responsibilities

• Assess patients and create proactive care plans.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment and adjust plans as needed.
• Collaborate with RN case managers in an integrated care team
