Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatrics-focused primary care experience. You will improve outcomes for long-term care residents by collaborating with a care team and case managers.

Expect a strong benefits/pay package and opportunities for career growth. Our APRN/NP or PA providers work mainly in nursing homes or assisted living with residents enrolled in Medicare Advantage plans.

Qualifications

• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Active APRN/NP or PA license in good standing.
• Experience with Electronic Health Records and standard MS Office tools.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, procedures, and consults.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers within an integrated care team.
