Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to join our team delivering primary care in long-term care settings. You will collaborate with physicians, case managers, and facility staff to improve quality of life for residents enrolled in Medicare Advantage plans.

Qualifications

• Nurse practitioner (NP) or physician assistant (PA) with geriatrics focus preferred
• Masters degree or higher required for APRN/PA licensure
• FNP/AGNP/AHACNP or PA license required
• 3+ years of clinical nursing or geriatric rehab experience
• Experience with Electronic Health Records (EHR) and Microsoft Office

Responsibilities

• Assess patients’ medical and mental health needs and create a proactive plan of care
• Ensure compliance with local, state, and federal healthcare regulations
• Prescribe medications and order labs, diagnostics, and consultations
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with RN case managers as part of an integrated care team
