Job summary

TruHealth is seeking an advanced practice provider (NP/PA) to deliver primary care in nursing homes and assisted living communities. You will work with a team of clinicians to improve outcomes for long-term care residents and advance a national care model guided by quality and coordination.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Masters or higher degree in nursing or related field.
• 3 years' experience in geriatric care or long-term care.
• Experience with Electronic Health Records.

Responsibilities

• Assess residents' medical and mental health needs and create an individualized plan of care.
• Prescribe medications and order lab work, diagnostics, and consultations.
• Coordinate with RN case managers as part of an integrated care team.
• Ensure compliance with local, state, and federal regulations governing clinical services.
