Job summary

TruHealth is actively seeking an advanced practice provider with geriatrics focus to join our team in nursing homes or assisted living communities. You will deliver personalized, coordinated care that improves residents’ quality of life and reduces unnecessary hospital visits.

You will work with physicians, RNs and case managers to implement our national model of care in a compliant, patient‑centered approach.

Qualifications

• Masters degree or higher in nursing or related field.
• APRN/PA license is required.
• Degree from an accredited APRN or PA program.
• At least 3 years of experience in geriatric nursing or rehab.
• Experience with Electronic Health Records and MS Office suite.

Responsibilities

• Provide coordinated primary care for long‑term care residents as part of a care team.
• Collaborate with physicians, RNs and case managers to optimize outcomes.
• Prescribe medications and order labs and consultations as appropriate.
• Monitor treatment efficacy and adjust plans as needed.
• Support facility staff training and integration of care processes.
