Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will work in nursing homes or assisted living communities, collaborating with a team of providers and case managers to deliver coordinated, person-centered care.

Requires an APRN/PA license, Masters or higher, 3 years in geriatric clinical nursing or rehab, EHR experience, and MS Office skills.

Qualifications

• APRN or PA license required.
• Masters degree or higher.
• 3 years’ experience in geriatric clinical nursing or rehab.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
