Zusammenfassung

Mankato Clinic is seeking a Geriatric Nurse Practitioner or Physician Assistant to join the Extended Care team in North Mankato, MN. This full-time, hybrid role focuses on proactive geriatric care within skilled nursing facilities, collaborating with physicians and care team members to manage complex conditions.

You will conduct assessments, develop treatment plans, and coordinate care with a multidisciplinary staff to improve outcomes for older adults.

Qualifikationen

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current Minnesota license or eligibility to obtain licensure.
• Board certification through AANP, ANCC, NCCPA, or applicable certifying organization.
• Ability to obtain Minnesota DEA certification.
• Wound Certification preferred.
• Minimum two years of clinical experience in family practice, internal medicine, geriatrics, or related setting.
• Geriatric experience, training or strong interest preferred.
• Valid driver's license and auto liability insurance.
• Ability to travel locally to area skilled nursing facilities.

Aufgaben

• Provide comprehensive primary and geriatric care to residents in skilled nursing facilities.
• Conduct health assessments, screenings, follow-up visits and routine evaluations.
• Diagnose and treat acute illnesses, chronic diseases and changes in patient condition.
• Develop, implement and adjust individualized treatment plans with the nursing home physician and care team.
• Provide health promotion, disease prevention, education and counseling to patients and families.
• Order, perform, supervise and interpret diagnostic studies within scope and regulations.
• Prescribe pharmacologic and non-pharmacologic treatments.
• Coordinate care with nursing home staff, physicians, specialists and therapists.
• Consult with, collaborate with, or refer to other providers as indicated.
• Support transitions of care and continuity across settings.
• Contribute to clinical excellence, teamwork and compassionate service.

Ausbildung
