Job summary

Mankato Clinic is seeking a Geriatric Nurse Practitioner or Physician Assistant to join the Extended Care team in Minnesota. You will provide primary and geriatric care at skilled nursing facilities, perform health assessments, and collaborate with nursing home staff, physicians, families, and other professionals.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current Minnesota license or eligibility to obtain and maintain licensure.
• Board certification through AANP, ANCC, NCCPA, or applicable certifying body.
• Ability to obtain Minnesota DEA certification.
• Wound Certification preferred.
• At least two years of clinical experience in family practice, internal medicine, geriatrics, or related setting.
• Geriatric experience or strong interest preferred.

Responsibilities

• Provide comprehensive primary and geriatric care to residents in skilled nursing facilities.
• Conduct health assessments, screenings, follow-up visits, and routine clinical evaluations.
• Diagnose and treat acute illnesses, chronic diseases, and changes in patient condition.
• Develop, implement, and adjust individualized treatment plans with the nursing home physician and care team.
• Provide health promotion, disease prevention, education, and counseling to patients, families, and caregivers.
• Order, perform, supervise, and interpret diagnostic studies within scope and regulations.
• Prescribe pharmacologic and non-pharmacologic treatments.
• Coordinate care with nursing home staff, physicians, therapists, and other providers.
• Consult with or refer to other providers as needed; support transitions of care.
• Contribute to a culture of clinical excellence, teamwork, and compassionate service.
