Job summary

TruHealth is actively seeking an advanced practice provider (APRN/NP or PA) with a geriatrics focus to join our team serving residents in long-term care and senior communities. You will collaborate with physicians, nurses, and case managers to deliver personalized, coordinated care and improve outcomes.

Responsibilities include independent assessment, treatment planning, prescribing, and ordering tests, while ensuring regulatory compliance and participating in an integrated care approach with

Qualifications

• Masters degree or higher in nursing or related field
• 3 years of clinical nursing or rehab in geriatric populations
• APRN or PA license required
• Electronic Health Records experience
• Proficiency with Microsoft Word, Outlook and Excel

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care
• Ensure compliance with local, state, and federal agencies related to clinical services you provide
• Prescribe medications and order lab work, diagnostic procedures and consultations
• Monitor patients’ compliance and response to their treatment and modify plans accordingly
• Work with RN case managers as part of an integrated care team
