Zusammenfassung

TruHealth seeks an advanced practice provider (NP or PA) with geriatric care experience to join our team. You will deliver primary care in nursing homes or assisted living facilities, exercising independent judgement and collaborating with supervising physicians.

You will work closely with facility partners and health plan leadership to implement Treat in Place Protocols, ensure high-quality care, and prevent hospitalizations. A $5,000 sign-on bonus is offered as part of a competitive package.

Qualifikationen

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Aufgaben

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ progress and adjust treatment plans as needed.
