Job summary

TruHealth is recruiting an advanced practice provider (NP or PA) to deliver primary care within nursing homes and assisted living communities. You will exercise independent judgement while collaborating with supervising physicians to treat acute and chronic conditions.

The role emphasizes patient-centered care for geriatric residents, with a strong focus on preventing hospitalizations and coordinating with facility partners. A $5,000 sign-on bonus is offered, along with competitive benefits.

Qualifications

• APRN license or PA license required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Experience with Fee For Service models preferred.

Responsibilities

• Assess patients’ medical and mental health needs and provide a care plan.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and modify treatment plans as needed.
