American Health Partners’s TruHealth division seeks an NP or PA with geriatrics experience to care for nursing home and assisted living residents. You will use independent judgment, treat acute and chronic conditions, and collaborate with a care team to improve patient outcomes.

You’ll work in a setting focused on long-term care, implementing Treat in Place Protocols while ensuring regulatory compliance and delivering high-quality care.

Formación

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsabilidades

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patients’ response to treatment and adjust plans as needed.
