Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with geriatrics focus, to join our long-term care team. You will provide coordinated primary care within nursing homes and assisted living communities, improve outcomes, and collaborate with case managers to support residents.

You will exercise independent judgment with supervision as needed, prescribe medications, order tests, and participate in implementing our national model of

Qualifications

• Masters or better in nursing or related field.
• APRN/PA licensure or eligibility required.
• Experience in geriatric care or long-term care preferred.

Responsibilities

• Assess residents and develop care plans addressing medical and mental health needs.
• Prescribe medications and order labs, tests, and consultations.
• Coordinate with RN case managers and care team to optimize outcomes.
