Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric focus to improve outcomes for long‑term care residents. You will collaborate with a team of providers and case managers to deliver coordinated care that enhances quality of life while reducing unnecessary hospital visits.

Applicants should hold an APRN or PA license and have experience in geriatrics; EHR proficiency and MS Office skills are required.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and create a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
