Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. The role involves collaborating with a team of providers and case managers within nursing homes or assisted living communities enrolled in Medicare Advantage plans.

As an APRN or PA, you will exercise independent judgment, treat acute and chronic conditions, and coordinate care with facility partners and health plan leadership to implement our national model in

Qualifications

• APRN/PA license required.
• Masters degree or higher.
• 3 years of clinical nursing or rehab in geriatric populations.
• Experience with electronic health records (EHR).

Responsibilities

• Assess patients' medical and mental health needs and plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order labs, diagnostic procedures and consultations.
• Monitor patients' progress and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
