Job summary

HarmonyCares is expanding its home-based primary care team in Texas, delivering in-home primary care to patients with complex healthcare needs. This role focuses on patient-centered care with minimal on-call requirements and a structured M-F schedule.

The Nurse Practitioner will manage 6–8 patients daily, use Athena AI Scribe for charting during the workday, and serve communities from Round Rock to Temple, including Georgetown and Belton.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high acuity patients in home settings.
• Collaborate with care team to address patient needs and coordinate care.
• Utilize Athena AI Scribe to complete charting within the workday.
• Manage patient panels across multiple Texas cities.
