Job summary

HarmonyCares is seeking Advance Practice Providers (NP/PA) to deliver in-home primary care for geriatric patients Monday through Friday, with no weekends or evenings. Based in multiple states, you’ll join a physician-driven, team-based model and manage a dedicated patient panel in home settings.

The role emphasizes patient-centered care, time to address needs, and a supportive environment with resources like an assistant in select markets and company vehicle where available.

Qualifications

• Licensed as a NP or PA in the state where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide in-home primary care to geriatric patients.
• Manage chronic conditions within patient panel.
• Collaborate with interdisciplinary team to coordinate care.
