Job summary

HarmonyCares Home Health is seeking a Nurse Practitioner to join our value-based in-home primary care team. The role supports patients with complex health needs, focusing on quality care over volume, and operates Monday through Friday, with appointments in Round Rock, Georgetown, Belton, Jarod, and Temple area.

Expect a salary starting at $135,000 per year plus a $7,500 sign-on, competitive benefits, and a comprehensive CME/licensing stipend.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver's license and good driving record

Responsibilities

• Provide primary care to high-acuity home health patients in their homes Monday to Friday.
• Chart and coordinate care to manage chronic conditions during the work day.
• Collaborate with care team to ensure patient-centered, quality outcomes.
