Job summary

HarmonyCares in East Dallas is seeking a dedicated Nurse Practitioner to deliver in-home primary care for high-acuity geriatric patients, Monday through Friday, with no weekend or evening shifts.

Join a physician-driven, team-based model that prioritizes patient-centered care over volume. Expect minimal call, home visits, and solid support including an assistant in select markets and a company car with driver in eligible areas.

Qualifications

• Licensed NP in the state of practice.
• DEA license/eligibility required.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Valid driver’s license with good driving record.

Responsibilities

• Provide in-home primary care to geriatric patients in the home health setting.
• Focus on patient-centered care, taking time to address each patient’s needs rather than volume.
