AlignMed Partners is seeking a Full Time Advanced Practice Provider (NP or PA) to float between three centers in Baltimore, MD: Franklin Woods Center, Advanced Rehab at Autumn Lake, and Hammonds Lane Center.

Qualifications

• Graduate from an accredited nursing program with RN license and MSN, concentration in Older Adult/Family NP.
• NP must be certified and licensed; PA must be ARC-PA graduate with PANCE and state licensure.
• DEA registration for each state of practice; NCCPA certification required.

Responsibilities

• Provide primary and urgent care to nursing center residents in collaboration with other providers.
• Collaborate with Center Leadership and Medical Director to meet center goals.
• Deliver high-quality care in alignment with AlignMed Partners' mission.
