Job summary

TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care within nursing facilities and assisted living communities, focusing on geriatrics. You will work with a team of providers and case managers to improve residents’ quality of life while reducing hospital visits.

You will operate with independent judgment under supervision, implementing TruHealth’s national care model in compliance with regulations and coordinating with health plan leadership for optimal outcomes.

Qualifications

• APRN or PA license required.
• Master's degree or higher in Nursing or related field.
• 3 years of experience in geriatric or rehab settings.
• Experience with EHR and MS Office.

Responsibilities

• Assess patients and develop a comprehensive care plan.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient response and adjust treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
