Job summary

TruHealth seeks an advanced practice provider (NP or PA) with geriatric/primary care experience to join our team in long-term care settings. You will collaborate with physicians and case managers to improve quality of life for residents enrolled in Medicare Advantage plans.

Responsibilities include assessing patients, prescribing medications, ordering tests, and coordinating care while adhering to federal and state regulations. Competitive pay, benefits, and growth opportunities available.

Qualifications

• Master’s degree or higher in nursing or physician assistant studies.
• Current APRN or PA license in good standing.
• Experience working with geriatric populations in clinical settings.
• Proficiency with Electronic Health Records and basic MS Office tools.

Responsibilities

• Assess patients' medical and mental health needs and plan care.
• Prescribe medications and order labs and consultations.
• Coordinate with RN case managers as part of an integrated care team.
• Ensure compliance with local, state, and federal regulations.
• Collaborate with facility staff to implement the model of care.
