Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with geriatrics or primary care experience to join our team serving residents in nursing homes and assisted living communities. This role emphasizes independent clinical decision-making and collaborative practice with supervising physicians.

You will assess medical and mental health needs, prescribe medications, order labs, and coordinate care with facility partners to improve

Qualifications

• APRN or PA license required.
• 3 years’ clinical nursing or rehab experience in geriatric populations.
• Electronic Health Records experience.
• Proficient with MS Office applications (Word/Outlook/Excel).

Responsibilities

• Assess patients’ medical and mental health needs and create a plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor and adjust treatment plans based on patient response.
