Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatric primary care experience to improve outcomes for long-term care residents. You will work in nursing homes or assisted living with a team of providers and case managers, offering coordinated, person-centered care.

The role involves independent clinical judgment, collaboration with facility partners, and adherence to federal/state regulations while delivering high-quality care and advancing career growth.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years geriatric clinical experience in nursing or rehab.

Responsibilities

• Assess medical and mental health needs with proactive care plans.
• Prescribe medications and order labs, imaging, and consults.
• Coordinate with RN case managers within an integrated care team.
• Ensure compliance with local, state, and federal regulations.
