Job summary

Total Life is seeking Nurse Practitioners for in-person longevity visits in the North San Diego County area. You will conduct geriatric assessments, review medications, and support cognitive health within a collaborative team.

You will travel to partner senior living communities, present to residents, and document encounters to Medicare standards using our EHR. Prior geriatric care and Medicare experience are valued for success in this role.

Qualifications

• Active, unrestricted California RN license and NP certification.
• Active NP Furnishing Number from California BRN.
• Current national NP board certification.
• Experience with Medicare documentation and E/M coding.
• Ability to travel to partner senior living communities in North San Diego County.

Responsibilities

• Conduct 45–60 minute longevity visits based on geriatric assessment.
• Evaluate eligible residents for weight management and GLP-1 therapy where appropriate.
• Coordinate follow-up care with primary care providers and specialists.
• Document every encounter in the EHR to Medicare standards.
• Represent Total Life professionally with residents, families and staff.
• Travel to assigned senior living communities in North San Diego County.
