Carease Health in New York is seeking a dedicated Nurse Practitioner to provide preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

You will work with facility staff to deliver patient-centered care, review documentation, and participate in ongoing education to stay current with industry best practices.

Qualifications

• Active NP license in the state of practice.
• 1+ year of experience in geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.
• Strong patient-centered approach and ability to work in a fast-paced environment.

Responsibilities

• Provide comprehensive preventative care to residents in skilled nursing facilities, utilizing RPM/CCM/CoCM strategies.
• Build relationships with facility staff and patients with a patient-centered approach.
• Document patient care efficiently with a dedicated scribe for support.
