Job summary

Carease Health is seeking a compassionate Nurse Practitioner to join our team in a hybrid role serving residents in skilled nursing facilities. You will provide primary and preventive care on-site and via remote monitoring, supported by our scribe team for documentation.

You will collaborate with our clinical group to develop personalized care plans, maintain accurate records in our documentation system, and build strong relationships with facility staff and residents.

Qualifications

• Active NP license in the state of practice.
• Minimum 1 year of experience in geriatrics or internal medicine.
• Excellent communication, collaboration, and teamwork skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide primary and preventive care to residents in skilled nursing facilities.
• Conduct on-site visits and remote monitoring using RPM and CCM technologies.
• Collaborate with our clinical team to develop and implement personalized care plans.
• Maintain accurate and timely documentation using our documentation system.
• Build strong relationships with facility staff and residents to ensure seamless care transitions.
• Participate in ongoing training and professional development to stay up-to-date on best practices.
