Carease Health, a nationwide preventive healthcare company, seeks an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with geriatrics/internal medicine background to deliver high-quality preventive care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote documentation supported by a dedicated scribe team.

You will build strong relationships with facilities, maintain a patient-centered approach, and collaborate with clinical leadership.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care or internal medicine.
• Experience with documentation review and charting.

Responsibilities

• Deliver high-quality preventative care to residents in skilled nursing facilities.
• Collaborate with facility staff and scribe team to ensure efficient documentation and care coordination.
