• 1+ year of experience in geriatric care and documentation review.

Responsibilities

• Provide high-quality preventive care to residents in skilled nursing facilities via on-site visits and remote work.
• Demonstrate a patient-centered approach, building strong relationships with residents and facility staff.
• Collaborate with the clinical team to align with facility processes and protocols.
• Conduct thorough assessments, diagnoses, and treatment plans for patients.
• Utilize RPM and CCM to monitor vital signs and detect abnormalities.
• Develop and implement individualized care plans to promote physical and mental well-being.
• Participate in quality improvement initiatives to minimize hospitalizations and improve patient outcomes.
