Carease Health – PA is seeking part-time Nurse Practitioners (21 hours per week) to deliver preventive care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote monitoring, supported by a scribe team for efficient documentation.

Active NP or PA licensure in the state, 1+ year geriatrics experience, and strong communication and teamwork are required. The role offers flexible hours, generous PTO, free CEUs, and referral bonuses from a collaborative team.

Responsabilités

• Provide high‑quality preventative care to residents in skilled nursing facilities through on‑site visits and remote monitoring.
• Collaborate with facility staff to develop and implement patient care plans.
• Maintain accurate and timely documentation of patient visits and treatments.
• Build strong relationships with facility staff and patients.
• Participate in quality improvement initiatives to enhance patient care standards.
