Carease Health - WI is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with geriatrics/internal medicine background to deliver preventative care to residents in skilled nursing facilities. The role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of geriatrics experience.
• Excellent communication, collaboration and teamwork skills.
• Masters-level education in Nursing or a related field.
