Medical Home Network is seeking a Geriatric Advanced Practice Provider to deliver high-quality, patient-centered care to older adults across our Springfield and Decatur, IL partner health centers. You will lead preventive geriatric services, AWVs, and ACP discussions, while helping design and scale programs for vulnerable communities.

Formación

• Active NP license (IL) and national certification.
• Geriatric care experience is required.
• Experience conducting AWVs and ACP discussions preferred.
• Strong communication skills and comfort with sensitive conversations.
• Experience in FQHCs or community health settings preferred.
• Experience building or scaling clinical programs and working in value-based care.

Responsabilidades

• Provide comprehensive primary and preventive care to aging patients in MHN’s contracted centers.
• Perform assessments focusing on chronic disease, function, cognition, falls, and meds.
• Develop individualized care plans with patients, caregivers, and inter-disciplinary teams.
• Conduct Medicare AWVs with risk assessments, cognitive/depression screens, fall risk, medication reconciliation, and referrals.
• Lead ACP discussions and document advance directives and POLST/MOLST as appropriate.
• Collaborate with care teams to ensure ACP preferences are followed across settings.
• Contribute to design, implementation, and scaling of geriatric care programs and workflows.
• Partner with operations, clinical leadership, and quality teams to improve outcomes and patient experience.
• Work with physicians, nurses, care managers, social workers; educate patients and families; supervise Medical Assistants.

Educación
