Job summary

Medical Home Network in Chicago is seeking a Geriatric Advanced Practice Provider to deliver comprehensive geriatric care within MHN’s network of health centers. You will conduct Annual Wellness Visits and ACP discussions, and help design scalable geriatric programs across under-resourced communities.

Ideal candidates bring IL NP/PA licensure, geriatric care experience, and proficiency with AWVs, ACP, and interdisciplinary care teams. Bilingual Spanish is preferred.

Qualifications

• Active NP license (IL)
• National NP certification (AANP or ANCC) or PA certification
• Experience in geriatric care required
• Experience conducting AWVs & ACP discussions
• Strong communication skills
• Experience in FQHCs or similar community health settings preferred
• Prior experience building or scaling clinical programs
• Experience with primary care or population health
• Background in value-based care, risk adjustment, or quality initiatives
• Familiarity with EHR documentation & Medicare compliance
• Experience working with interdisciplinary teams
• Fluent Spanish speaker required

Responsibilities

• Deliver comprehensive primary and preventive geriatric care to diverse populations
• Conduct AWVs and ACP discussions with patients
• Develop and scale geriatric care programs and workflows
• Collaborate with physicians, nurses, care managers, social workers, and teams
• Support quality metrics and documentation for value-based care
• Educate patients, caregivers, and staff on preventive care and aging topics
• Directly manage Medical Assistant(s) for clinical operations
