Job summary

Medical Home Network is seeking a Geriatric Advanced Practice Provider to deliver high-quality, patient-centered geriatric care at partner health centers across the Chicago metro area. You will conduct AWVs, ACP discussions, and collaborate with care teams to design scalable programs focused on preventive and age-friendly care.

You will lead care management, document accurately, and train staff while expanding clinical programs in under-resourced communities.

Qualifications

• Current, unrestricted NP or PA license in Illinois.
• Master’s degree or higher from an accredited NP/PA program.
• Experience delivering geriatric primary and preventive care.
• Proven ability to conduct AWVs and ACP discussions.
• Experience building or scaling clinical programs is a plus.
• Experience in FQHC or community health settings preferred.
• Fluent in Spanish.

Responsibilities

• Provide comprehensive primary and preventive care to seniors enrolled with MHN’s partner centers.
• Perform assessments focusing on chronic disease, functional status, cognitive health, fall risk, and med management.
• Develop individualized care plans with patients, caregivers, and interdisciplinary teams.
