Matter Health is seeking a Nurse Practitioner to support older adults in Chicago, IL. You will manage a dedicated patient panel as their ongoing primary care provider, traveling to affordable housing communities and patients' homes to deliver care.

The role offers clinical autonomy with collaboration from an interdisciplinary team and Medical Director for complex cases. Telehealth and in-person visits combine to improve access.

Formación

• Graduate degree (Master’s or Doctorate) from an accredited NP program.
• Active, unrestricted APRN license in Illinois.
• National board certification appropriate to patient population served.
• 3+ years of NP clinical experience.
• BLS certification is required.
• Valid driver's license and reliable transportation.
• Experience with EHR documentation.

Responsabilidades

• Provide ongoing primary and preventive care to older adult patients.
• Participate in rotating after-hours on-call schedule.
• Manage and grow a dedicated patient panel in assigned communities.
• Deliver patient-centered care in community settings.
• Collaborate with interdisciplinary teams and Medical Director.
• Support preventive care, chronic disease management, and care coordination.
• Utilize telehealth and in-person care delivery models.
• Maintain accurate EHR documentation with coding specificity.
