Job summary

Medical Home Network is seeking a Geriatric Advanced Practice Provider to deliver high-quality, patient-centered care for seniors at partner health centers in Springfield and Decatur, IL. The role emphasizes preventive geriatric care, chronic disease management, and care coordination within an interdisciplinary team.

The position involves conducting AWVs, ACP discussions, and developing individualized care plans to improve outcomes and align with value-based care initiatives.

Qualifications

• Active NP license in IL and national certification (AANP/ANCC) or PA certification.
• Experience in geriatric care is required.
• Experience conducting AWVs and ACP discussions is required.

Responsibilities

• Provide comprehensive primary and preventive geriatric care to patients enrolled in MHN’s contracted health centers.
• Perform AWVs and ACP discussions in collaboration with care teams.
• Develop individualized care plans and coordinate with patients, caregivers, and interdisciplinary teams.

Skills

Strong communication skills

Fluent Spanish (plus)

Education

Master’s degree or higher in NP/PA program

Current, unrestricted NP/PA license in IL
