Matter Health is seeking a Nurse Practitioner to provide ongoing primary and preventive care for older adults in Chicago, IL. This fully community-based role requires building lasting relationships with patients in housing communities and patients’ homes, with a high degree of clinical autonomy and collaboration with an interdisciplinary team.

The position emphasizes equity and patient-centered care, with telehealth and in-person care options, competitive salary, and full-time benefits.

Competenze

• Graduate degree (Master’s or Doctorate) from an accredited Nurse Practitioner program.
• Active, unrestricted APRN license in Illinois.
• National board certification as a Nurse Practitioner (Family, Adult, Gerontology, or Adult-Gerontology).
• 3+ years of NP clinical experience, preferably with aging or medically complex populations.
• BLS certification.
• Valid driver’s license and reliable transportation (car stipend provided).
• Experience with EHR documentation (athenahealth preferred).
• Commitment to equitable, patient-centered care.
