Job summary

Matter Health is seeking a Nurse Practitioner to support older adults in Atlanta, GA. You will serve as their ongoing primary care provider, managing a dedicated panel within community housing settings.

This fully community-based role involves travel across the market to deliver care in homes and affordable housing communities, using telehealth and in-person visits with strong clinical autonomy and collaborative support from an interdisciplinary care team.

Qualifications

• Active, unrestricted NP license.
• Board certification: NP (AANPCB or ANCC) in family, adult, geriatric, or adult-gerontology.
• Active DEA license and CS registration as required.
• 3+ years of NP clinical experience, aging or medically complex populations.
• BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide ongoing primary and preventive care to older adult patients.
• Manage and grow a dedicated patient panel within communities.
• Deliver patient-centered care in community settings.
• Collaborate with interdisciplinary teams and Medical Director.
• Support preventative care, chronic disease management, and care coordination.
• Utilize telehealth and in-person care delivery to improve access.
• Maintain accurate EHR documentation with coding specificity.
