Matter Health is seeking a Nurse Practitioner to support older adults in Detroit, MI. You will be the ongoing primary care provider for a dedicated panel, delivering patient-centered care across homes and housing communities.

You will collaborate with an interdisciplinary team and a Medical Director while maintaining strong relationships with patients and families. The role emphasizes clinical autonomy and after-hours on-call coverage.

Qualifications

• Graduate NP degree from an accredited program.
• Active, unrestricted Michigan NP license.
• National board certification (AANPCB or ANCC) for geriatrics/family.
• 3+ years of NP clinical experience with aging/complex populations.

Responsabilités

• Provide ongoing primary and preventive care to older adults.
• Travel across Detroit, in-home and community settings.
• Manage a dedicated patient panel with care coordination.
• Maintain accurate EHR documentation and coding specificity.
• Collaborate with interdisciplinary teams and Medical Director.
