Job summary

Pine Park Health is seeking a Clinical Director (NP) to lead a growing team of Nurse Practitioners and Physician Assistants providing in-home, value-based care to seniors. This hybrid role blends direct patient care with provider leadership across multi-site operations.

Ideal candidates will bring geriatric expertise, a track record in team management, and strong clinical decision-making skills to drive preventive care, care coordination, and provider performance in a patient-centered,

Qualifications

• Active unrestricted DEA with schedules 2, 2N, 3, 3N, 4, 5.
• Master's degree in Nursing with Family or Adult-Gerontology specialization.
• Minimum three years of specialized geriatric experience in clinical, community or senior living settings.
• Minimum 3 years of direct people management experience supervising NP/PA or similar clinical staff.
• Demonstrates exceptional clinical acumen including advanced diagnostic skills.
• Ability to work autonomously in fast-paced, multi-site healthcare environments.

Responsibilities

• Lead and develop a team of 6-8 Nurse Practitioners and Physician Assistants.
• Ensure providers meet metrics including visit targets and care gaps closures.
• Optimize provider schedules and panel assignments.
• Drive community growth by engaging with senior living facilities.
• Lead quality improvement initiatives to enhance patient care.
